IMPORTANT- New Immunization requirement for 7th-12th graders!

boy vaccine.jpgWe ask that you please read the following letter with great care.  It contains vital enrollment information for ALL OUSD students in grades 7-12 for the 2011-12 school year.

During the past year, the state of California experienced widespread cases of “pertussis,” also known as whooping cough.  To help contain the spread of this highly contagious condition, ALL 7-12 grade students MUST show definitive proof of immunization (Tdap) against this infectious disease before the start of the 2013-14 school year.

By law, all students in grades 7-12 will NOT be able to register or attend class this fall without proper documentation supporting proof of immunization.

OUSD Wellness Policy

If your child needs to take any form of medication at school, we must have the district form filled out with the parent’s and doctor’s signature (these required forms are in the Health Office).  This is for all medication, including over-the-counter medication and cough drops.
